Many patients are discharged from physical or occupational therapy still experiencing residual weakness, deconditioning, movement compensation, or fear of re-injury. Without structured progression, these patients may plateau, regress, or return with preventable setbacks. My role is to serve as a bridge between rehabilitation and long-term functional independence, offering continuity of care once formal therapy concludes.
